Self Pay Fees
At Seaside Family Medicine we believe in providing high quality, affordable care to all patients. We have two options for payment: pay as you go or the direct access primary care membership model.
Pay As You Go
With this model, the type of office visit you are requesting determines the cost of your service. Payment is due at the time of the service in the form of credit card, cash, personal check or CareCredit.
Below is a basic list of self pay fees, please do not hesitate to contact the office if you have any questions or do not see the service you need listed.
Membership Model
With this model patients pay a low monthly fee of $70 a month to include your care for the year. The $70/month is auto-charged to your credit card on file at the beginning of each month. The following is included in the membership model:
Annual check up with routine screening labs through Quest
If female, annual well women visit with pap smear
Unlimited sick visits including any necessary in-house diagnostic testing
Unlimited followups/chronic disease visits
If diabetic, A1C every 3 month is included
Televisits (if medically appropriate)
Access to the patient portal to message your care team
